If there are any changes since your last visit, it is important that you fill, update, and sign before your next visit.  This helps us promote your safety by minimizing forms handling and other administrative hand contact during your visit, (sorry for the extra work — HIPAA rules do not allow us to pre-fill personal information on the online forms)
